Instructions

These chit sheets are used by Branch Officers, Shop Stewards and Members to itemize expenses. They are turned in to the Branch Trustees for their approval.

There are 2 forms per page, since the form is turned in in duplicate. They are in .pdf format (you need Adobe Acrobat 4.0 or above). You can resize the forms on your browser to easier fill them out on your computer. Fill out the left form and the right one fills out on its own. Then you can print them, you must set your printer to print in landscape style, rather than portrait style. Please seperate the 2 sheets prior to submitting chit to the Trustees.
